This page explains the limited information used by this website and its callback request.

Callback requests

The callback request asks only for your name, phone number, and permission to call. It deliberately has no space for health information. Anything about your condition is best discussed with Jennifer on the phone.

The form is part of this website rather than an external service, so what you type is sent to this site first and stored in its database, hosted by Cloudflare. It is then forwarded to Google Forms, where responses are collected in a private Google Sheet, so a request is never held in only one place. Google’s privacy terms apply to the forwarded copy. Your phone number and name are the only details sent on; the country your request came from is recorded alongside the copy held by this site, and your IP address is used briefly to limit repeated submissions.

Website analytics

This site uses Google Analytics to understand general website use. Visitors from the EU, UK and Switzerland are asked first. Form values are not sent to Analytics, and advertising signals and enhanced form tracking are disabled. You can turn analytics off at any time in your analytics preferences.

Error reporting

This site uses Sentry to report technical errors. Error reports are configured to exclude form contents, URL query strings, user identity, and session replay.
